What Causes Bad Breath? The Complete Guide
Bad breath is caused by volatile sulfur compounds (VSCs) — gases produced when bacteria in your mouth break down proteins. Most of those bacteria live on the back of your tongue and in your throat, which is why brushing your teeth alone often doesn't fix it.
That single sentence explains most of what people find confusing about bad breath: why it comes back an hour after brushing, why mints stop working, and why someone with excellent dental hygiene can still worry about it.

What is bad breath, exactly?
Halitosis is the clinical term. In roughly 85–90% of cases, the source is inside the mouth — not the stomach, and not the teeth themselves.
The smell comes from three main gases:
- Hydrogen sulfide — the rotten-egg note
- Methyl mercaptan — closer to rotting cabbage; the one most associated with gum disease
- Dimethyl sulfide — a sweeter, more chemical smell, often from sources outside the mouth
These are produced by anaerobic bacteria — bacteria that thrive without oxygen. They feed on proteins, specifically the sulfur-containing amino acids cysteine and methionine, which come from food debris, dead cells, and mucus. The gases are the waste product.
So bad breath isn't dirt, and it isn't a hygiene failure. It's a byproduct of bacterial metabolism.
Where do the bacteria actually live?
Four places, in rough order of how much they contribute:
1. The back of the tongue
The single biggest source. The rear third of the tongue has a rough, grooved surface that holds a coating of bacteria, dead cells and food residue. It's low-oxygen, protein-rich, and almost never cleaned — most people brush their teeth and stop.
2. The throat and tonsils
Tonsil crypts trap debris that hardens into tonsil stones, which are densely populated with the same anaerobic bacteria. Breath from this source can't be reached by brushing or rinsing at all.

3. Between the teeth and under the gumline
Where gum disease is present, methyl mercaptan production rises sharply. This is the one source a dentist can directly treat — which is why persistent bad breath is worth a dental check.
4. A dry mouth
Not a bacterial site itself, but a multiplier. Saliva is antibacterial and physically washes debris away. Less saliva means more bacteria, faster.

Brushing does what it's designed to do — it cleans tooth surfaces. But tooth surfaces are a small fraction of the mouth, and they aren't where most odor-producing bacteria live.

What makes bad breath worse?
Anything that dries your mouth
Sleep, mouth breathing, dehydration, many medications, alcohol, caffeine, and alcohol-based mouthwash all reduce saliva. This is the mechanism behind morning breath: saliva flow drops to near zero overnight and bacteria multiply undisturbed for hours.
Coffee and alcohol
Both dry the mouth. Coffee also leaves behind sulfur-containing compounds of its own, which is why the effect is immediate rather than gradual.

Garlic and onion — a different mechanism entirely
Garlic breath isn't caused by residue in your mouth. Allyl methyl sulfide is absorbed into the bloodstream during digestion and released through your lungs for up to a day afterward. How can you tell if you have bad breath?
Poorly, is the honest answer. You adapt to your own smell within minutes — a phenomenon called olfactory fatigue — so self-assessment by cupping your hands and breathing into them tells you almost nothing.
More reliable signals:
- Lick the inside of your wrist, wait ten seconds, and smell it. Saliva carries the VSCs and your nose hasn't adapted to your wrist.
- Scrape the back of your tongue with a spoon and smell the residue.
- Notice behavior: people stepping back, offering gum, or angling away during close conversation.
- Ask someone who will tell you the truth. Uncomfortable, but by far the most accurate method.

The useful distinction is between masking odor and reducing what creates it.
Masking

Frequently asked questions
Does bad breath come from the stomach?
Rarely. The esophagus is normally closed, so stomach gases don't continuously reach the mouth. Around 85–90% of bad breath originates in the mouth itself. Reflux can contribute in some people, but it's the exception rather than the rule.
Why is my breath worse in the morning?
Saliva production drops to almost nothing during sleep. With nothing washing them away, bacteria multiply for seven or eight uninterrupted hours. Morning breath is close to universal and is not a sign of a problem.
Does mouthwash cure bad breath?
It reduces it temporarily. Alcohol-based mouthwashes have a complication: the alcohol dries the mouth, and a dry mouth produces more odor. Some people find their breath is worse an hour after rinsing than before.
Can I smell my own breath?
